We’ve created CLUB ONE for the Patients that have NO insurance, as a way for them to have access to Dental Procedures with significant discounts and small annual fees. No deductibles, no waiting periods, no annual maximums, no denials of claims or pre-approvals. We offer two (2) Plans: The Preventative Plan and The Periodontal Plan.

1.- PREVENTIVE PLAN

For patients with basic cleanings and absence of periodontal disease.

  • Adult Membership (18 and Older)
  • Children Membership (10 to 17 years old)

One (01) Adult $299* per year. SAVE 40% !

Two (02) Adults $499* per year.

Family Plan: Add $229* per each child per year.

*A small, PPE (Personal Protective Equipment) fee will be added to your cost. – D1999

  • Professional dental cleanings (2 per year) – D1110/D1120
  • Doctor exams (1-2 per year) – D0120
  • Unlimited Single PA x-rays (As determined by your dentist) – D0220/D0274
  • Fluoride treatment (1 per year) – D1206
  • One emergency exam per year (if needed) – D0140
  • 10% Discount off all standard implant and surgical fees.
  • 15% Discount off all other treatments fees (some exclusions may apply)

2.- PERIODONTAL PLAN

For patients enrolled in active periodontal maintenance due to prior treatment of periodontal (gum) disease.

  • Adult Membership (18 and Older)
  • Children Membership (10 to 17 years old)

Per Person (Adult and Child): Add $299* to the Preventive Plan, per year. SAVE 35% !

*A small, PPE (Personal Protective Equipment) fee will be added to your cost. – D1999

  • Perio maintenance cleanings (3 to 4 per year) – D4910
  • Doctor exams (1-2 per year) – D0120
  • Unlimited Single PA X-rays (As determined by your dentist) - D0220/D0274
  • Fluoride treatment (1 per year) – D1206
  • One emergency exam per year (if needed) - D0140
  • 10% Discount off all standard implant and surgical fees.

What forms of payment are accepted?

  • All forms of credit and debit cards including Visa, Mastercard, Discover and American Express.

    Note: CareCredit cannot be used.

When can I start using my membership?

  • Your membership is effective immediately!

How do I schedule my preventive care?

  • Simply call our office and setup an appointment for yourself or your family members and please do not forget to mention to the front desk representative that you are a CLUB ONE Member.

How can I use my Member Exclusive discount for restorative, elective or cosmetic treatment?

  • Simply schedule an appointment with our office and be sure to tell us you’re a CLUB ONE Member. When you arrive, we’ll make sure your membership is “current” and you will be eligible for instant savings off our regular fees.

How do I know if I need the Perio Plan?

  • If you’re a current patient, please contact us to confirm you’re on Perio Maintenance. If you are a new patient, we will make a recommendation after your initial exam.

What happens if I miss an appointment?

  • It is your responsibility to schedule your appointments and keep them within the 12 month time period; if you don’t make your appointments in that period, you forfeit all the benefits of the Plan. Please see our office policy regarding no-shows and last minute cancellations.

Can I add members to my account?

  • Absolutely! Your entire family can join.

Can I transfer my membership to another dentist?

  • Your CLUB ONE membership is solely with our office and as such is non-transferrable. If you move to another area, you may cancel your membership at any time.

If I cancel, can I get a refund on my membership?

  • No. Refunds will not be provided for dues paid under any circumstances, including failure to schedule and maintain appointments.

What information is required for me to sign up for my CLUB ONE membership?

  • An email address for receipts, credit or debit card, full name, address, phone and date of birth.

Legal and Disclaimers

  • Our membership plan is not (a) insurance, (b) a qualified health plan under the Affordable Care Act, (c) pre-payment for future services, or (d) payment for access to discounted services. Rather, our membership plan is a direct payment arrangement for services, products and care provided. Your payments must be current to receive the services and discounts included in the membership plan.
  • Payment for treatment not included in the membership plan is due at the time of service.
  • The membership plan benefits may not be combined with insurance or any other offers or discounts.
  • Our membership plan does not cover procedures completed by specialists outside our practice.
  • The membership plan runs for 12 months and includes an auto renewal feature. You can opt out of the auto renewal at any time from the Member Portal. You will receive an email or a text message reminder 45 days prior to the renewal date.
